From everything I've read it usually is the same price as the price of a one-day ticket. Currently, $63 (?). Beverages, I think, are separate (certainly the booze is).

A 4 year old might have a better time at Ariel's Grotto. I have 2 little girls and I do not think I would take them to Club 33. Not being judgemental or anything just saying she might have a better time at another resteraunt.

They usually raise the price when they raise admission prices, which I think they just did recently. The minimum to eat at the Club is usually the same price as what it costs for a one day park pass to Disneyland.
I remember the first time I had lunch at the Club is was $36.
I went a few months ago, and I brought around $100 with me. The price of the meal was just around 60 dollars, but with a drink, tax, and tip, it turned out to be eighty-two dollars. Which was great, just enough for me to buy a shot glass!
